Why 40% of Men Have This Problem and Almost None Talk to a Doctor About It

In over a decade of practice, I have noticed a pattern with gynecomastia patients. They typically arrive later than almost any other patient group. Not because they did not notice the problem years earlier. Because they spent years convincing themselves it was not serious enough to address.

By the time most men walk into my clinic in Mumbai, they have changed how they dress, stopped going to the beach, trained around the problem in the gym, and quietly arranged their life to avoid situations where it would be noticed. The average time between first noticing gynecomastia and seeking a consultation is 3 to 5 years, sometimes longer.

That delay has a cost. Not just personal. Medical.

What gynecomastia actually is

Gynecomastia is the enlargement of glandular breast tissue in men. Not fat. Glandular tissue. This distinction matters enormously because glandular tissue does not respond to diet, exercise, or weight loss. You cannot train it away. You cannot fast it away. The men who spend years doing extra chest work at the gym and then arrive at my clinic frustrated are not failing to try hard enough. They are using the wrong tool for the problem.

Many cases also involve some degree of localised chest fat alongside the glandular component. But the firm, disc-like mass you can feel underneath the nipple area is glandular tissue. That requires a different kind of treatment.

Why does it happen?

The most common cause is a shift in the balance between oestrogen and testosterone. This happens naturally during puberty, which is why many teenage boys develop temporary breast tissue. In most cases it resolves on its own within 1 to 2 years. When it does not, or when it develops in adult men, the causes include medications (certain antidepressants, heart medications, and anabolic steroids are common offenders), hormonal conditions, or sometimes no identifiable cause at all.

Before any surgical conversation, I run blood work. If there is a treatable hormonal cause, that needs to be addressed first. Operating on active hormonal gynecomastia carries a real risk of the condition returning after surgery.

The silence around it

Gynecomastia affects an estimated 40 to 60 percent of men at some point in their lives, making it one of the most common male conditions in plastic surgery. And yet the social conversation around it is almost nonexistent compared to the actual prevalence.

Women's body confidence is discussed constantly in media. Men's body image, particularly around conditions like gynecomastia, gets almost no airtime. What this produces is a large number of men who have no reference point for whether what they are experiencing is normal, treatable, or worth addressing.

Most of my gynecomastia patients assumed for years that they were in a small, unusual minority. Learning that it is medically common does not eliminate the personal difficulty of it. But it does remove the layer of shame that delays the decision to seek help.

When is surgery the right answer?

Established gynecomastia that has been present for more than 12 to 18 months is very unlikely to resolve on its own. At that point, the glandular tissue is mature and will not shrink without surgical removal. The procedure involves removing the gland through a small incision hidden at the lower border of the areola. Where fat is also present, liposuction is done at the same time. Most patients go home the same day.

Recovery is straightforward. A compression vest for 4 to 6 weeks, return to desk work within 7 to 10 days, gym in 4 to 6 weeks. The result is permanent. The gland, once removed, does not regrow.

The patients who wait the longest

I want to say something directly to the men reading this who have been sitting with this problem for years.

The delay is understandable. But it is also a choice with consequences. Every year of wearing loose shirts and avoiding shirts entirely, of engineering your life around a medical condition that is treatable in a 90-minute procedure, is a year lived differently than it had to be.

That is not a judgment. It is just an observation from someone who sees the before and after of this decision regularly. The men who finally act on it are consistently more surprised by how straightforward the process is than by anything else.
